its same as vanilla, only loud portion is point of return.
And yes, enemies do spawn, your in the middle of the city what did you expect?

I also improved some stuff:

• Mayhem Difficulty (and above) will spawn an additional ene_security_3 at the main door.
• Guards during combat, prefer to defend an area rather than rushing at the threat.
• Point Of No Return (Main feature)
