Tweaked version of Bedebao's Simple Health Thresholds texture mod. I didn't really like the original colours, so I modified them a bit.

Does not include the Stamina Circle seen in the screenshots. The screenshots also show the health circle as semi-transparent, this is because I modified it using MUI.
